Referrals and Admissions
Atrium Medical Center’s mission is “
to provide long term acute care with respect and dignity, in partnership with physicians, families and other care providers in our community
We work closely with hospitals and other healthcare facilities and providers throughout our region to ensure that patients referred for our services are accommodated efficiently and effectively. Patients can be referred from:
Short term acute care hospitals
Skilled nursing facilities/sub-acute units
Assisted living centers
Acute rehabilitation facilities
Patient’s homes through house call physicians or home health agencies
Managed care/insurance case managers
Referrals made to Atrium Medical Center for evaluation are handled in a timely manner by an assigned clinical liaison. The clinical liaison will:
Complete an on-site assessment
Verify eligibility for insurance
Determine clinical appropriateness for admission
Assess service or equipment needs
Obtain authorization from health plans (if necessary)
Meet with the patient and their family to share information, answer questions and schedule a tour
Assists in transferring the patient once the patient is medically stable for transfer and consents to admission to Atrium Medical Center
Atrium Medical Center accepts Medicare and many commercial insurance plans. Hospitalization insurance information is obtained prior to admission. The Admissions Department works closely with all insurance plans to verify coverage benefits policy limitations and obtain authorization. As new contracts are regularly signed, please contact the hospital to see if a plan is covered.
For those patients covered by Medicare as their primary insurance, Part A covers a semi-private room, meals, general nursing, and other hospital services and supplies. During your stay, your care manager will work with you to keep you informed regarding co-payments, deductibles, etc.
